Use this register before deploying an AI-assisted business workflow. The purpose is to identify cases that cannot safely follow the standard path and make ownership, evidence, authority and recovery explicit.
For every exception, record the trigger, affected outcome, source evidence, missing or conflicting context, decision owner, permitted actions, escalation path, response target, completion signal, and whether the result was verified, disputed or remains unknown.
The register supports implementation and evaluation. It does not prove that an AI system is safe, compliant, accurate or valuable. Test positive, negative, stale, missing-data and unauthorized-action cases in conditions similar to the deployment environment.

- Name the observable trigger and affected business outcome.
- Attach the source records and state what is missing or disputed.
- Name the decision owner, permitted actions and escalation path.
- Set a realistic response target and a completion signal.
- Record whether the result is verified, disputed or still unknown.
